Access to Department of Biomedicine, BBB

The department have laboratories and areas which have restrictions due to access, and we aim to secure that our users have an agreement with the department describing the access conditions, including documentation that information due to health, environment and security regulations and evacuation procedures are communicated.

Access will be granted on an individual basis, and we therefore need you to fill in this form.

Please do the following:

1. Fill out the attached form to the best of your ability
2. Get a signature from your contact person at IBM (can also be done electronically, by email)
3. Deliver the form together with a copy of your work contract/master contract, at the reception, 6th floor
