5. Scientific activity

A. Urinary incontinence (UI) (Ongoing PhD project)

Below I present my plans for completing my PhD work. I have taken the liberty to enclose drafts of papers for my PhD degree as part of the 10 papers to be judged by the committee, instead of including only published papers in other areas not relevant to my thesis.

For my PhD-thesis, I will be using urinary incontinence as a case for clinical epidemiology issues, diagnostic reasoning, classic epidemiology and validation of Internet as a research tool.

Project title: Evidence based diagnosis of urinary incontinence

As of today, I have the data I need for my PhD –thesis in two different SPSS-databases, 2 published papers (1 international) and several drafts for papers to be published within the next 6-12 months. My PhD project consists of data collected from two projects. My mentors are prof. dr. med. Steinar HunskÂr and dr. med. Hogne Sandvik.

Validation of a questionnaire for detecting urinary incontinence

Project 1 is a clinical epidemiology study of the use of a structured questionnaire for detecting UI compared to a gynaecological assessment including urodynamics as a gold standard. I started this research in 1993 on a project with professor dr. med. Steinar HunskÂr at Div. for General Practice, entering, coding and analysing diagnostic data from 250 women with urinary incontinence (UI), referred from GPs for consultation with a urotherapist, urodynamic assessment, and a gynaecological work-up. This work has so far resulted in two published papers, 1 national and international publication. All data are collected and in SPSS format.

PAPER I describes the findings from this specialist clinic, and is thus a descriptive, base-line paper.

PAPER II validates a previously validated preoperative score for UI (DIS), and presents a ROC-diagram, which stimulated my interest in learning about evidence-based medicine.

The University of Bergen, as part of my Lecturer post, funded this project.

WEB-EPI UI: Validation of Internet as a research tool

Project 2 is probably the first study in the world to examine the validity of Internet-collected data with data collected in a classic epidemiology study, HUNT, where the data have been published under the EPINCONT umbrella. All data are collected, and coded in SPSS-format.

PAPER III (Draft) will describe the findings compared with HUNT-data, using the same questionnaire as in EPINCONT.

PAPER IV (Draft) is a web-based study, also including a questionnaire (ICIQ-UI SF) from Bristol Urological Institute.

PAPER V (Planned) a systematic review of the diagnostic value of appropriate tests (symptoms, signs and tests) for detecting the various types of UI, and applies the methods from the JAMA series of papers on ≥The rational clinical examination≤ http://www.emory.edu/WHSCL/grady/inetgrp/ratclin.html.

Project 2 had no funding, apart from the generosity of the web site owners and the time spent by my co-researchers.

B. Evidence based medicine (EBM) (1997- )

Teaching EBM, writing letters and commentaries have been important for my own methodology development. Collaborating with colleagues like professor Arild Bj¯rndal, and tutoring at workshops led by professor Trisha Greenhalgh at the University College London, Andy Oxman at the National Institute of Public Health (Folkehelsa) in Oslo, and with Toby Lipman at the Durham Workshop has been very stimulating, and important with respect to future networking.

Evidence based case report (2002)

While working at Medical dept., Deaconess Hospital, in 1996 I drafted a paper/case report on evidence treatment of pulmonary oedema in heroin addicts.

Evidence-basing well-baby clinics (2002)

I was invited to write a chapter on professional development and EBM for ≥Helsestasjonsboka. This book targets health visitors and GPs working at well-baby clinics, and was published August 2002.

National guidelines for antenatal care in Norway

I chaired the group working on national guidelines for antenatal care: Klovning A, Backe B, Roland B, Eide B, Holan S, Mathisen M, et al. Nye nasjonale retningslinjer for gravide. Oslo: Sosial- og helse direkoratet; 2005.

D. Evidence based laboratory medicine (EBLM)

Quality assurance of laboratory tests in general practice (1996-1999)

I started on a 4-year university grant 01.05.97, researching quality assurance and rational use of laboratory medicine, but got entangled into evidence-based medicine, and then in e-health.

For this 4-year University grant I planned to prepare a thesis on "Aspects of external quality assurance in primary health care: from analytical quality control to quality assurance of test interpretation and evidenced based guidelines," with principal mentor being Prof. dr. med. Sverre Sandberg. At times life brings on a multitude of challenges, so I decided to quit my stipend 3 Oct 1999 to work full time for NettDoktor. This project has led to 1 international publication, and several national publications. As of 29.09.02 I have decided to concentrate on the urinary incontinence project for my PhD project, and after that I will probably take up some of the projects I started on in NOKLUS.

The National Academy of Clinical Biochemistry is the Academy of the American Association for Clinical Chemistry (AACC) and is dedicated to advancing the science and practice of clinical laboratory medicine through research, education, and professional development. The Academy is a leading advocate for scholarship through scientific research and it conducts educational programs to promote scientific discovery, application, and integration. It publishes Laboratory Medicine Practice Guidelines (LMPG) for the application of clinical biochemistry to medical diagnosis and therapy.

In this Laboratory Practice Medicine Guideline (LMPG) the National Academy of Clinical Biochemistry (NACB) is examining the application of evidence-based medicine to the form of diagnostic testing known as Point-of-Care testing (POCT.) For the purpose of this document POCT is defined as ≥clinical laboratory testing conducted close to the site of patient care, typically by clinical personnel whose primary training is not in the clinical laboratory sciences or by patients (self-testing).

E. e-Health

CMWR research projects (2000- )

Since Mid-September 2000, more and more of my work has been directed towards research again, leaving the editorial and administrative functions of NettDoktor behind. This already has given birth to a number of projects under Centre for Medical web Research, like e.g.:

Sleep disorders research (2000- )

This e-survey collected n=414, data ready for analysis. This was performed using Inquisite, a web-based survey and statistics program. There were 414 respondents the few days that the questionnaire was accessible. There was a selection bias towards having sleep disorders, as expected. Data show how Net users self-rate their sleep disorders compared with Norwegian guidelines for defining sleep disorders. Data collection is over, and a paper is planned.

Winter depression (2000- )

An e-survey, with data already collected using the validated SPAQ questionnaire for diagnosing winter depression. This was also performed using Inquisite.

F. Research on educational issues

IKT-basert undervisning/web-based course in EBM (2000- )

The validation of this was designed as a randomised, controlled trial, where this module was programmed autumn 2000, and evaluated by a questionnaire on paper and web, triangulated with a focus group. http://www.nettdoktor.no/uib/ (username aklovning, password ak321). A paper is planned, and should document my interest in IT-based learning, and competence in evaluating teaching processes.

I was responsible for the preparation of a report describing the function and organising of a National Centre for Primary Care Acute Medicine.

Elevated sedimentation rate and immunoglobulins (ESR & IgA, 1988)

Fredrik Bass¯e, initially studying elevated sedimentation rate and Immunoglobulins (ESR and IgA) in general practice. I presented a poster and gave a 10 minute presentation at the conference "MIE-88: Medical Informatics Europe '88", held in Oslo, 17-20 august 1988, presenting these data. The paper was published in "Lecture notes in Medical Informatics".

Medical Informatics (1985-88)

Further on, we worked on the concept of developing a computerised medical record that could be used as a research tool. This was planned to generate data for clinical decision-making, using techniques from artificial intelligence (AI), like high-level programming languages such as PROLOG and LISP.

In June 1986, I participated at the WONCA 86 - 11th Conference of the World Organisation of National Colleges, Academies and Academic Associations of General Practitioners/Family Physicians in London 1-6 June 1986, presenting 2 posters based on 2 abstracts by dr. med. Carl-Fredrik Bass¯e: PROMED: PROBLEM-ORIENTED COMPUTERIZED MEDICAL RECORD and A Combinatorial Diagnostic System for General Practice.

I hold a one-year degree in Information Science (Informasjonsvitenskap grunnfag). I manage Macintosh and Windows/DOS computers, networking, www etc. proficiently.

I have completed a one-year part-time compulsory course in university teaching ("Utviklingsprogram i universitetspedagogikk"). I write and speak fluent English. My teaching may be divided into evidence-based medicine and general practice, with a developing focus on e-health. At the end of this document, I present a chronological listing of my teaching.

EBM at the University of Bergen

1997: Facilitated a long-term EBM-group at my own department. Developed the web for ≥Finding answers to questions in evidence-based medicine≤. I have since been responsible for the EBM-introduction at the end of the fifth year for medical students, and designed/hold the EBM/EBHC-course for final year Medical students at the University of Bergen, together with prof. Steinar HunskÂr. Medical journalism/EBM

From October 1998 medical journalist presenting research data for a biweekly Norwegian medical journal DagensMedisin. Topics covered: Donepezil & dementia. ACE-inhibitors, urinary incontinence, migraine, evidence-based quality indicators, beta-blockers for heart failure, the HOT-study, should TCA or SSRI be chosen for treatment of depression amongst many others, altogether 19 texts.

All NettDoktor content is popularised medical information for lay people, altogether about 6 000 pages, and should speak for itself: www.nettdoktor.no, equal to www.netdoktor.dk and www.netdoctor.co.uk. I was Editor-in-Chief for NettDoktor.no until 31.12.00. In Norway, traffic figures showed that there were between 60.000 and 100.000 unique users per month. In NetDoktorπs 9 countries, NetDoktor served altogether 1 million unique users.

Dagens Medisin

My work for Dagens Medisin documents my efforts to critique and present findings of current research to colleagues, acknowledging that information in medical journals is often difficult to grasp. By using EBM-tools, I have presented critically appraised information to colleagues.

I have held many administrative positions as documented below. During my time as the Managing Director of NettDoktor (adm. dir.), I had the responsibility for the payrolls of 120 persons in Norway, 8 full-time, 25 part-time and about 90 contributors to the web site. I was student representative for the DAU-komiteen (data-assistert undervisning) working on a new study plan for the University of Bergen. Richard Fosse, Fred Holsten og Hallvard LÊrum were the committee members. Our mandate was to investigate on the use of computers in medical teaching, leading to a report. This was presented as "Demonstrasjon av noen av de funksjoner som er beskrevet i rapporten fra DAU-komiteen" at Haukeland University Hospital 15. desember 1988 for members of the study plan committee (SPK) and the extended study plan committee (SPK-U). I had programmed ≥Pain≤ in HyperCard to demonstrate how computers could be used in medical teaching.

I was the administrative leader of a 5-doctor practice at Ulriksdal legesenter, in Bergen, with day-to-day management of personnel. I participated in a management programme.

When I was offered to be in charge of launching NetDoktorπs first foreign country launch of the Danish web site providing access to EBM-resources, I decided to put my PhD project on hold. I was in charge of the start-up of the Norwegian arm of Europeπs leading health-website, www.nettdoktor.no, launching a site for professionals June 25^th 1999 (www.nettdoktor.no/pro) and then launching the lay-people site, www.nettdoktor.no on October 18th 1999. The Norwegian staffing comprised of 8 full-time and 120 part-time employees, before downsizing. In July 1999, I participated in the initial founding rounds of NettDoktor with one of Europe¥s largest health care investors, APAX http://www.apax.com/, in Munich. NettDoktor consisted of two webs www.NettDoktor.no and NettDoktor Pro www.nettdoktor.no/pro (username line99 and password line99). NettDoktor is a health web for lay people, whereas NettDoktor Pro provides access to evidence-based resources for health. As of today, NetDoktor is Europeπs largest health web with 1 million unique users reading 14 million pages per month in 9 countries.

Jeg veiledet en gruppe p 13 studenter fra kull 89 i en oppgave som tar for seg de mulighetene EDB gir oss som leger i et informasjonssystem. Gruppen har tredelt arbeidet i en kort orientering om maskinvaren, bruk av Internet for lesing av informasjonsbaser ved hjelp av Mosaic, og endelig et overblikk over telemedisinske muligheter. Gruppen presenterte arbeidet for resten av kullet 17. juni 1994 som en forlesning med praktisk demonstrasjon av nettet ved hjelp av LCD skjerm koblet til datamskin.	Medical students
