Et dilemma i mange fattige land er om en skal gi god behandling til få, eller dårlig behandling til mange. Av 86.000 nyfødte barn i Tanzania med hivpositive mødre ble ca 50 prosent behandlet med forebyggende medikamenter i 2009. (FN-rapport fra Tanzania 2010).
